At Nuform Health, we are committed to delivering a high-quality, physician-led experience. Please review our refund policy carefully, as it differs by service type.
Annual Physician Fee
- The physician fee is fully refundable up until the time of your consultation.
- If your provider determines that you are not a candidate for therapy following your consultation, the physician fee will be fully refunded.
- Once your consultation has been completed and treatment has been recommended, the physician fee is non-refundable.
Medications
- All medication orders are final at the time of purchase.
- Once a medication has been ordered with the pharmacy, we are unable to provide a refund.
- Medications cannot be returned to the pharmacy or clinic once dispensed.
Blood Work & Diagnostic Labs
- Once blood work has been completed, we are unable to issue a refund.
- Blood work is refundable up until the time of completion.
